The game features frequent bizarre and disturbing imagery and can be played in both 2D and VR modes. The most pertinent scene to the rating involves the player entering a theatre, filled with a crowd wearing animal masks. As the player approaches the stage, a jaunty piano tune starts as men in the same masks begin to fall from the ceiling, suspended in the air with nooses around their necks. The men will struggle and kick their legs, making faint noises. If the player does nothing, they will fall still and silent, but the player can release the men from their nooses if they act in time. However, all the men that the player saves will simply be replaced by another hanging victim. All the while the crowd is watching and reacting, laughing and cheering as the player struggles to keep up with the falling bodies, or gesturing angrily once they give up.
