The player’s human character fights against a range of mutated humanoid characters and fantastical demonic creatures. Attacks are both physical and magical, and responses are often exaggerated, with unrealistic effects. Enemies may disappear when defeated or, in the case of the mutated humanoids, they may be dismembered with limbs and bones detaching before disappearing, with black blood shown. The strongest violence occurs when a humanoid creature is impaled through its forehead, causing the character to split in two, with visible blood and gore.

Throughout the game, the mutilated and charred corpses of humans and livestock can be found in almost every level.
