This game contains depictions of strong, realistic-looking violence towards human characters. Splashes of blood are seen when the player is hit, while they flinch and yell in pain before falling to the ground. Fantasy characters react in a similar realistic way when attacked; they bleed, jerk backwards and moan in pain. Upon death, fantasy characters fade out of existence. If the player character is killed, they will fall to the ground before the screen goes black and the character is prompted to quit or to re-spawn at their last checkpoint.

This game also features strong gory images. Dead bodies litter the environments and exhibit gore. While exploring the environment, players will often encounter piles of dismembered limbs and bodies. Large amounts of blood and gore can be seen in these scenes. There are also scenes in which the player is chased and is relentlessly threatened with imminent death as a consequence of being caught.
