The game features violence against a vulnerable and defenceless character and depictions of strong violence. It focuses on a child-like character, who often meets many gruesome ends, such as being sucked through a fan and being bisected/disembowelled, or falling from a great height and being impaled on a spike. Acts of violence against the protagonist also include adult characters spotting, sitting on top of and strangling him, or him being mauled by dogs. Violence may be accompanied by red blood splatters.
